Not every project needs to be complex to carry real purpose, and this is a good example of that. Built in PHP as part of a college assignment, this form was designed to simulate a real-world need: supporting the social services department of an outpatient clinic. Simple in structure, but sharp in purpose, a hands-on exercise in how technology can connect with real social contexts.
